Meskwaki Settlement School is excited to announce that the Meskwaki Tribal Health Clinic is able to provide COVID-19 vaccinations to students ages 16 and older.

 

Due to FDA guidelines, only students who will be 16 years of age on or before Friday, May 7 will be eligible to participate in the COVID-19 vaccination clinic.

 

Students will be receiving the Pfizer vaccine, which will be given in two separate doses approximately 21 days apart.  Dose one will be administered on Friday, May 7; with dose two being given on Friday, May 28.  Both doses will be administered at school during Warrior time.

 

Meskwaki Tribal Health Clinic will collect general demographic information, as required by the CDC and in order to report this vaccine in IRIS (Immunization Reporting Information System).

 

Parental consent is required. Please see the attached consent form, vaccination information document, and pre-vaccination screening forms.

 

Consent forms must be returned to school no later than 05/01/2021. There will be paper copies of the consent form available in the office if you are unable to print this document at home.
